Education verification for the initial related degree and most recent degree
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year of start
Negative 10-panel drug screen within 30 days of start
Two verified references within 1 year of start
Skills checklist, updated annually and not expiring within 30 days of start
TB questionnaire, updated annually
Immunization or immunity documentation for measles, mumps, rubella, hepatitis B, varicella, tetanus/diphtheria/Tdap, and flu vaccination or approved exemption
Completion of required onboarding and confidentiality documents
Occupational health appointment before start
