Physical Therapist - Home Health (Travel/Contract)

Location: San Jose, California
Contract Length: 16 weeks (approximately 4 months)


Job Overview

  • Specialty: Physical Therapy in Home Health setting
  • Patient Focus: Home Health, Wound Care, Geriatric patients
  • Visits: Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
  • Weekend Requirement: No weekends required
  • On-Call: Not required (discretionary but not mandatory)
  • Locals: Accepted within a 50-mile radius
  • Mileage Reimbursement: IRS standard from first patient to last patient (not to/from home)
  • Dress Code: Any color scrubs, closed-toe shoes
  • Charting: Must be completed onsite using Kinnser/Wellsky; no charting allowed at home
  • Parking: Free residential parking available

Required Qualifications

  • Valid Physical Therapist license in California
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • Basic Life Support (BLS) certification required at time of submission

Compliance & Documentation Requirements

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form prior to start
  • Background checks within 1 year, including SS Trace, County searches (7 years), Nationwide, Sex Offender registry
  • Annual EPLS/GSA/SAM and HHS/OIG checks
  • Physical exam within 6 months signed by MD/DO, confirming freedom from active communicable/infectious diseases
  • Immunizations/vaccinations or exemptions required for:
    • COVID-19 (vaccine or medical/religious exemption)
    • Influenza (vaccine or declination form during flu season)
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Measles, Mumps, Rubella (proof of immunity or vaccination)
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (titer, vaccination, history of chickenpox, or declination)
  • Negative TB test or chest X-ray within 1 year (with specific rules for past positives)
  • 9-panel drug screen within 1 year of start (instant drug screen acceptable)
  • Auto insurance card required
  • Skills checklist and competency exams as applicable, including medication/pharmacology competency if relevant
  • References: 2 professional references required for travel/allied/per diem acute care; 1 for per diem non-acute care

Additional Notes

  • Contract length is 4-6 months; less experienced clinicians must commit to 6 months (non-negotiable)
  • Compliance documentation must be complete before starting patient care; compliance processing may take 2-3 weeks
  • All required time off (RTO) requests must be submitted at time of application; no additional RTO accepted after offer
  • Territory and county coverage details will be discussed during the interview; may include surrounding counties
  • Valued clinician qualities: patient care focused, punctual, self-starter, diligent with completing charting onsite using provided tablet
