Physical Therapist - Home Health (Travel/Contract)

Location: Pomona, California
Contract Length: 13 weeks

Job Details

• Specialty: Home Health Physical Therapy
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
• Guaranteed 40 hours per week
• Patient load: 5-7 patients per day, minimum 6 visits daily or 30 points weekly
• Types of cases: Geriatric patients
• Territory covers up to a 60-mile radius including Yorba Linda, Orange, Anaheim Hills, and Santa Ana
• Dress code: Scrubs (any color), no jeans
• iPad provided for documentation; cell phone use not allowed during work
• Mileage reimbursed at IRS standard rate

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of recent experience as a Physical Therapist
• BLS (Basic Life Support) certification required (AHA, ARC, or Military accepted)

Compliance & Background Requirements

• Background checks within 90 days of start, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education verification (highest degree related to profession)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting requirements vary by state (not applicable for California)
• Employment history verification for 7 years required
• Professional references: 2 within 2 years for travel/allied/per diem acute care
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Agency Background Attestation required prior to start

Health & Immunization Requirements

• Physical exam within the last 12 months (physician statement)
• Fit test for N95 mask (no specific model required)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB Questionnaire required
• Immunizations or documented immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

• Proof of current auto insurance required due to driving responsibilities
• No on-call duties
