Physical Therapist - Travel/Contract Position

Location: Hartford, Kentucky
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks

Work Schedule

• Monday-Thursday: 9:30 AM - 6:00 PM (30 min lunch, 2 paid 15-min breaks)
• Friday: 8:30 AM - 5:00 PM (30 min lunch, 2 paid 15-min breaks)

Requirements

• Doctorate of Physical Therapy (DPT) or equivalent
• Ability to work with both in-patients and out-patients

Certifications & Licenses (Required Prior to Start)

• ACLS (Advanced Cardiac Life Support) – AHA only (Instructor/Provider/RQI acceptable)
• BLS (Basic Life Support) – AHA only (Instructor/Provider/RQI acceptable)
• PALS (Pediatric Advanced Life Support) – AHA only (as applicable)
• State License Verification (Primary source verification)
• National Certification/Verification (as applicable by profession and state)
• NURSYS Verification (for RN, LPN/LVN, AP as applicable)

Background Checks (Required Prior to Start)

• SS Trace and County searches for the last 7 years of locations lived/worked/in school (within 1 year of start)
• EPLS/GSA/SAM
• HHS/OIG

Competency Exams (As Applicable)

• Medication Test/Pharmacology Competency Exam
• Other Competency Exams relevant to profession/specialty

Health & Immunization Requirements

• Physical Exam within the last 12 months
• Negative TB test or Chest X-Ray (within 1 year; chest X-ray within 5 years if positive PPD)
• TB Questionnaire (if positive TB)
• Immunizations or proof of immunity (Measles, Mumps, Rubella, Hepatitis B, Varicella) or declination forms
• Drug Screen within the last 12 months
• Seasonal Vaccinations or Declinations for Flu and COVID (with mask requirements if declined)
