Job Opportunity: Therapy/Rehabilitation (Physical Therapist or Speech-Language Pathologist)

Position Details
- Specialties Needed:
- Physical Therapists (PTs) in Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanilaus, Sacramento Counties
- Speech-Language Pathologists (SLPs) in Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma/Marin Counties
- Patient Load: Focus on patients per week rather than per day
- Visits Required: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
- Types of Cases: Home Health, Wound Care, Geriatric Home Health Patients
- Experience Required:
- Minimum 1 year in Home Health
- Minimum 2 years overall as a clinician
- First-Time Travelers: Accepted
- Weekend Requirement: No weekends required
- On-Call: Not required (discretionary)
- Territory: May include surrounding counties; discussed during interview
- Locals Accepted: Yes, within a 50-mile radius
- Mileage Reimbursement: IRS standard from first to last patient visit (not to/from home)
Licensure & Certifications
- Must have valid State License and BLS certification at time of submission (no pending licenses accepted)
- ACLS and PALS certifications preferred (American Heart Association or American Red Cross)
- Other certifications as applicable to profession
Work Environment & Policies
- Dress Code: Any color scrubs, closed-toe shoes
- Charting System: Kinnser/Wellsky; charting must be completed onsite (no remote/charting from home)
- Parking: Free residential parking available
- COVID & Flu Policy:
- COVID vaccination required (religious declination allowed)
- Flu vaccination optional with declination form required
- Compliance: All required documents must be submitted and approved before starting; compliance process may take 2-3 weeks
- Return to Office (RTO): All RTO requests must be submitted with application; no additions after offer
Contract Length
- 4-6 months per contract
- Less experienced clinicians (minimum experience not met) must commit to 6 months (non-negotiable)
- (Note: Contract length interpreted as 26 weeks based on provided data)

Required Documentation & Compliance (Summary)
- Government-issued photo ID (Driver’s License/State ID)
- Emergency Contact Form
- Background Checks (7-year search including SS trace, county searches, sex offender registry)
- EPLS/GSA/SAM and HHS/OIG checks (annual)
- Certifications: BLS (required), ACLS, PALS (preferred)
- State License verification (primary source)
- Competency Exams as applicable
- Auto Insurance card (if applicable)
- Physical Exam within 6 months (signed by MD/DO, free of communicable diseases)
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or chest X-ray (within 1 year)
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccination, or history)
- Drug Screening: 9-panel within 1 year
- Seasonal Vaccinations: Flu (or declination), COVID (or exemption)
- References: 2 professional references for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
