Job Opportunity: Therapy/Rehabilitation - Travel/Contract

Location: Alameda, California
Contract Length: 13 weeks (approximately 3 months)
Shift: Tuesday - Saturday
Weekends: Yes
Float: As needed to other site locations on weekends if there is a dire need

Experience & Certifications Required

• Minimum 2 years of Acute Care experience
• Required Certifications:

  • Basic Life Support (BLS)
  • Physical Therapy (PT) certification

• Backfill position
• May require floating to other sites on weekends in urgent situations

Education Verification

• Verification of highest level of education required
• Must meet minimum education requirements for the position

• ACLS (Advanced Cardiac Life Support) - AHA only
• PALS (Pediatric Advanced Life Support) - AHA only
• NRP (Neonatal Resuscitation Procedure)
• CHEMO (Chemotherapy)
• TNCC (Trauma Nursing Core Course)
• EFM (Electronic Fetal Monitoring)
• Management of Assaultive Behavior (MAB)
• NIH Stroke Certification
• CPI (Crisis Prevention Institute)
• State License (current and unencumbered)
• NURSYS Verification for RN, LPN/LVN, AP
• Tech Certifications as applicable (e.g., Radiology, Mammography)
• Orientation Safety Manual Acknowledgement

Competencies

• Medication Test/Pharmacology Competency Exam (as applicable)
• Color Blind Testing (Relias or Ishihara exam within 90 days)
• Competency Exam (as applicable)

Facility Specific Requirements

• Passport-style photo for badge (no filters)
• Completion of Alameda Health System competencies and New Employee Orientation modules on first day
• Competencies must be completed by 11:59pm PST on first day; failure to complete within 14 days results in termination
• Re-orientation required if absent from Alameda Health System for more than 6 months

Health & Immunization Requirements (within 90 days of start unless otherwise noted)

• Physical Exam signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties
• Fit Mask test with approved masks (3M 8210, 3M 1860S, 3M 1860R)
• Tetanus/Diphtheria/Tdap vaccination within 10 years or signed declination (TD not accepted)
• Negative TB test or Chest X-Ray with annual updates
• TB Questionnaire (updated annually)
• Proof of immunity or vaccination for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (positive titer or declination)
  • Varicella
• Drug Testing: Oral saliva drug screen within 30 days of start (rapid tests not accepted)
• Flu Seasonal Vaccination or declination on file by November 6 (masking required during flu season Nov 1 - Mar 31)
• Color Blind Exam (within 90 days)

Employment History & References

• Current resume including last 7 years of employment
• Employment verification for past 3 years (3rd party acceptable)
• References required per agency standards (not facility requirement)
