Job Description

• Must be a Licensed Dental Assistant in the state of Minnesota
• Minimum 1 year of experience required
• Start Date: ASAP, with options for 10/20 or 10/27
• Work Schedule:

  • Monday to Friday, 7:45 AM start
  • Monday to Thursday end at 5:00 PM
  • Friday end at 1:00 PM
• No weekends or on-call required
• Candidate must live outside a 25-mile radius from the facility

Required Certifications

• BLS (Basic Life Support): AHA BLS certification required
• Other certifications may be relevant depending on role but not explicitly required for Dental Assistant

Licensure

• Must have valid Minnesota state license verification
• Primary source verification required

Competencies

• Competency exams required as applicable by profession, with an 80% passing score or meeting/exceeding the Angoff score
• Medication/Pharmacology competency exam required for RNs (not applicable for Dental Assistant)

Facility Requirements & Documents

• Photo for badge: Grey background, no selfies, JPG format
• Dress code: Review required prior to start
• Confidentiality agreement must be signed before starting
• Education modules completed during orientation (approx. 16 hours)
• COVID-19 information and protocols to be reviewed

Health & Immunization Requirements

• Physical exam within 1 year of start
• Fit mask (N95) required
• Tdap vaccination required (no religious declinations accepted)
• Negative TB test or chest X-ray per Minnesota state guidelines
• MMR (Measles, Mumps, Rubella) proof required (positive titer or 2 vaccines)
• Hepatitis B vaccination, positive titer, or declination accepted
• Varicella proof required (positive titer or 2 vaccines)
• Seasonal flu vaccination or declination (mask required if declined)
• COVID-19 vaccine preferred; medical/religious exemptions accepted
• Drug testing: 9 panel within 1 year of start (not required for client clearance)

Background & Screening

• Criminal background check within 1 year of start, including:

  • 7-year search, SS trace, counties, sex offender, nationwide
  • Education verification (not required for CNAs)
  • International searches if applicable
• EPLS/GSA/SAM and HHS/OIG excluded provider checks required
• Two references from recent employers within 2 years required (for non-clinical roles)
• Skills checklist required for all clinical candidates
• Education verification required (third party), except for CNAs
• Work history verification for non-clinical candidates (3 years)

Minnesota-Specific Requirements

• MN DHS NetStudy Fingerprint Questionnaire must be completed immediately after signing contract
• Candidates cannot start with fingerprints pending unless they have clearance or a “more time needed” letter
• Fingerprinting must be completed within 2 weeks of registration
• Candidates may need to travel to a livescan location if not available locally

Additional Notes

• Candidates must complete a time-sensitive demographic questionnaire for MN DHS fingerprinting
• Candidates must leave/return badges to manager on last day of assignment
• Modules and orientation details will be provided on start date
• COVID-19 religious and medical exemption forms require additional processing time (up to 3 weeks)
