Registered Nurse - Step Down

Location: Redlands, California

Contract Length: 13 weeks

Job Description

• Start date: May 5
• Shift: Single set of RTO up to 7 consecutive days considered
• Patient ratio: 1:3 with trachs and ventilator management required
• Cardiac drip management (non-titrated) including Amiodarone, Milrinone, Lasix, Cardizem (titrated), Dopamine, and Dobutamine required
• Insulin 3:1 preferred but not required
• Weekend requirement: Minimum 2 weekends per month (may be more)
• Float within areas of competency: Med/Surg, Telemetry, Observation
• Use of Omnicell medication system and SBAR communication
• 24-hour orientation provided
• Navy scrubs required (not provided)
• No local candidates within 50 miles considered
• Provider support includes Interpretation Services, Lift Teams, Pharmacy, Phlebotomy, Physical Therapy, Radiology, Rapid Response Teams, Respiratory Services, Social Services, Unit Secretary
• Intensivist and Hospitalist coverage available

Required Skills and Experience

• Experience in MS/Telemetry, Telemetry, PCU/DOU/IMC/Stepdown units
• Management and interpretation of dysrhythmias
• Telemetry unit monitoring
• Use of Rapid Response Teams
• Post-MI care
• Feeding tube care and management
• Pre/Post Hemodialysis management
• Renal failure care
• Accessing ports
• BPA (Best Practice Alerts)
• Central line and peripheral line care/management
• PICC line management
• Starting IVs
• Heparin protocols
• Management of cardiac drips (no titration)
• Management of vasoactive drips (no titration)
• Care of CVA patients
• Care of ventilated patients
• Chest tubes (Emerson/Pleur-evac)
• External CPAP/BIPAP
• High Flow Nasal Cannula (HFNC)
• Interpretation of ABGs
• Oxygen delivery via nasal cannula and face masks
• Non-invasive airway management
• Tracheostomy management
• Isolation precautions
• Wound care and wound vac management

Preferred Skills and Experience

• Post cardiac angiogram care
• IV insulin protocols
• Titration of insulin drips
• Titration of cardiac drips
• Pre/Post neurosurgery care

Certifications Required

• NIH Stroke Certification
• ACLS (AHA only; RQI not acceptable)
• BLS (AHA only; RQI not acceptable)

Additional Information

• Meditech experience preferred
• First-time travelers with PCU experience and 1:3-4 ratios will be considered
• Prior travel experience is not explicitly required
• Candidate must be able to float within competency areas
• Must be willing to work required weekends

Required Qualifications

• Valid Registered Nurse (RN) license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of Progressive Care Unit (PCU) experience within the last year
• Current ACLS certification issued by the American Heart Association
• Current BLS certification issued by the American Heart Association
• NIH Stroke Scale (NIHSS) certification
• Ability to manage ventilated patients with a 1:3 patient ratio including trach and ventilator management
• Management of non-titrated cardiac drips including Amiodarone, Milrinone, Lasix, Cardizem, Dopamine, and Dobutamine
• Experience with use of Rapid Response Teams
• Post-myocardial infarction (Post-MI) care
• Pre/Post Hemodialysis management
• Renal failure care
• Use and management of Best Practice Alerts (BPA)
• Central line care and management
• PICC line management
• Heparin protocol management
• Care of cerebrovascular accident (CVA) patients
• Experience with High Flow Nasal Cannula (HFNC) therapy

Compliance and Onboarding Requirements

• Background checks including SS trace, county searches, nationwide, sex offender, and international if applicable
• Drug screening (10 panel including THC) within 30 days of start
• Physical exam within last 12 months, updated annually
• Fit mask testing with approved masks prior to start
• Immunizations and titers: Tdap, TB testing, Measles, Mumps, Rubella, Hepatitis B, Varicella, Flu vaccination or declination, COVID vaccination plus booster or approved exemption
• Various facility-specific acknowledgments and signed agreements (e.g., confidentiality, code of conduct, abuse reporting)
• Photo for badge following dress code guidelines
• Skills checklist and competency exams as applicable

This position is ideal for experienced RNs with strong telemetry and stepdown unit skills, comfortable managing ventilated patients and complex cardiac drips, and willing to work a travel contract with weekend requirements.
