Location: Lacey, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Specialty: Home Health Occupational Therapy
  • Minimum 1 year recent OT experience required
  • OASIS experience required
  • EMR experience with Homecare Homebase (HCHB) required
  • Average caseload: 5-7 patients per day
  • Expected visits: 6 daily or minimum 30 points weekly
  • Guaranteed 40 hours per week
  • Patient population: Geriatric
  • Territory: Up to 60-mile radius from office/hub
  • Locals accepted at travel rate
  • Dress code: Scrubs (any color), no jeans
  • iPad provided for documentation; cell phone use not allowed
  • Mileage reimbursed at IRS standard rate
  • Timely communication and documentation required
  • No on-call or weekend requirements

Required Qualifications

  • Valid Occupational Therapist license in Washington
  • Minimum 1 year of recent Occupational Therapist experience within the last year
  • Basic Life Support (BLS) certification

Compliance & Background Requirements

  • Background checks within 90 days of start, including:
    • SSN Trace
    • County searches (last 7 years)
    • FACIS Level 1
    • Nationwide Sex Offender
    • Motor Vehicle Records (MVR)
    • Education verification
    • Work history verification (7 years)
  • EPLS/GSA/SAM check within 90 days of start
  • HHS/OIG check within 90 days of start
  • Fingerprinting as required by Washington state (on-site if candidate has not lived in WA for past 3 years)
  • State-specific criminal history and employability status checks as applicable
  • Education verification of highest degree related to profession
  • Signed Orientation Safety Manual Acknowledgement (updated annually)
  • NURSYS verification if applicable

Health & Safety Requirements

  • Physical exam within last 12 months (not required annually)
  • Fit mask test (N95 accepted)
  • Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
  • Immunizations or titers for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening: 9-panel within last 12 months
  • Seasonal flu vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required

Additional Requirements

  • Proof of current auto insurance required (due to driving responsibilities)
  • Two professional references within 2 years of start
  • Full 7 years of work history documented with no gaps
  • Annual skills checklist and competency exams as applicable
