Job Opportunity: Licensed Masters of Social Worker (LMSW) – Hospice
Location: Lacey, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday (5x8 hours)
Hours per Week: 40 hours
Job Description
- Specialty: Hospice Social Services
- Average caseload: 5-7 patients per day (geriatric cases)
- Electronic Medical Record system: HCHB
- Territory: Up to a 60-mile radius from the office; flexibility with territory expected
- Dress code: Scrubs (any color), no jeans
- Device provided: iPad (no personal cell phones allowed)
- Mileage reimbursed at IRS standard rate
- On-call: No
- First-time travelers: Not accepted
- Locals accepted at travel rate
Required Qualifications
- Valid Licensed Master Social Worker (LMSW) license in Washington
- Minimum 2 years of recent experience as a Licensed Master Social Worker, including experience within the last year
- Basic Life Support (BLS) certification (AHA, ARC, Military accepted)
Compliance and Background Requirements
- Background checks within 90 days of start, including:
- SSN Trace
- County searches (last 7 years)
- FACIS Level 1
- Nationwide Sex Offender
- Motor Vehicle Records (MVR)
- Education verification (highest degree related to profession)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Employment application with full 7 years of work history, no gaps
- Work history verification for 7 years (3 attempts required if unverifiable)
- Two professional references within 2 years of start
Health and Safety Requirements
- Physical exam within the last 12 months (physician statement)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB questionnaire required for all candidates
- Immunizations or titers required:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months of start (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
Additional Requirements
- Proof of current auto insurance required if job responsibilities include driving
- Signed acknowledgment of Orientation Safety Manual (updated annually)
- Completion of facility-specific Acceptable Use Policy prior to start
- Facility-specific flu and COVID declination statement if applicable
Notes
- Prior travel experience is required; first-time travelers will not be considered.
- Candidates must be flexible with travel territory within the specified radius.
- No personal cell phones allowed during work; iPads are provided for documentation and communication.
