Job Opportunity: Physical Therapist - Home Health
Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Discipline: Therapy/Rehabilitation
- Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1, may visit up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Start Date: ASAP
- Weekend Requirement: No
- On-Call: No
- Floating Between Units: No
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Pending License Accepted: Yes
- Transportation: Traveler must have own transportation; occasional travel up to 50+ miles
- COVID-19 Vaccination: Primary series plus one booster required
- Charting System: EPIC/Oasis (Oasis experience required)
- Attire: Any color scrubs
- Parking: Free
- Guaranteed Hours: Facility may call off up to one shift every two weeks
Required Qualifications
- Valid Physical Therapist license in Oregon (pending license accepted)
- Minimum 1 year of skilled home health experience within the last year
- Experience in a Home Health setting
- Experience with OASIS charting system
- Basic Life Support (BLS) certification required
Compliance and Documentation Requirements
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Immunizations and health screenings including:
- Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination during flu season
- Respirator fit testing required for travel candidates (updated annually)
- COVID-19 vaccine or declination required
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years
Additional Notes
- First-time travelers are not accepted for this position
- No weekend or on-call shifts required
- Facility groups clients to minimize drive time
- Facility may call off up to one shift per pay period (every two weeks)
- Must comply with all required background checks, immunizations, and certifications prior to start
- Modules and training requirements may apply as per regional and specialty-specific guidelines (e.g., WorkRamp modules)
