Job Opportunity: Physical Therapist - Home Health

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 26 weeks (approximately 6 months)

Job Description

  • Provide home health physical therapy services primarily in the Sacramento, Modesto, and Stockton areas.
  • Patient focus is on quality care rather than number of patients per day.
  • Minimum of 23-24 patient visits per week required to qualify for a full-time guarantee.
  • Types of cases include Home Health, Wound Care, and Geriatric Home Health patients.
  • No weekend work required.
  • On-call duties are discretionary and not required.
  • May be asked to cover surrounding counties.
  • Locals within a 50-mile radius are accepted.
  • Charting must be completed onsite using provided technology; no remote charting allowed.
  • Dress code: Any color scrubs and closed-toe shoes.
  • Free residential parking available.
  • COVID-19 vaccination required; religious exemptions allowed. Flu vaccination declination accepted with proper documentation.

Required Qualifications

  • Valid Physical Therapist license in California.
  • Minimum of 1 year of Physical Therapist experience within the last year.
  • Experience in a Home Health setting.
  • Speech Language Pathologist experience.
  • Basic Life Support (BLS) certification.

Compliance & Documentation Requirements

  • Government-issued photo ID.
  • Background checks within 1 year, including SS trace, county searches, and nationwide sex offender registry.
  • Annual EPLS/GSA/SAM and HHS/OIG checks.
  • Certifications: BLS required; ACLS and PALS preferred.
  • State license primary source verification.
  • Physical exam within 6 months prior to start, including clearance of communicable diseases.
  • Immunizations or exemptions required for:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella (per birth year guidelines)
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (titer, vaccine, history, or declination)
    • Seasonal Flu vaccination or declination during flu season
    • COVID-19 vaccination or approved exemption
  • 9-panel drug screen within 1 year.
  • Auto insurance card if using personal vehicle.
  • Emergency contact form.
  • Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care.
