Job Opportunity: Speech-Language Pathologist - Home Health

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: Approximately 26 weeks (6 months)

Job Description

  • Provide home health therapy services primarily in various counties including Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma, and Marin.
  • Patient caseload focuses on home health, wound care, and geriatric patients.
  • Minimum of 23-24 patient visits per week required to qualify for a full-time guarantee.
  • No weekend work required.
  • On-call duties are discretionary and not required.
  • Locals within a 50-mile radius are accepted.
  • Charting must be completed onsite using specified electronic health record systems; no remote charting allowed.
  • Dress code: Any color scrubs and closed-toe shoes.
  • Free residential parking available.
  • COVID-19 vaccination required; religious exemptions allowed. Flu vaccination is optional with declination.

Required Qualifications

  • Valid Speech Language Pathologist license in California.
  • Minimum of 1 year of Speech Language Pathologist experience within the last year.
  • Experience in a Home Health setting.
  • Physical Therapist experience.
  • Basic Life Support (BLS) certification.

Compliance and Documentation Requirements

  • Government-issued photo ID.
  • Background checks including SS trace, county searches, and national databases within the last year.
  • Annual verification of exclusion lists (EPLS/GSA/SAM, HHS/OIG).
  • Certifications: BLS required; ACLS and PALS preferred but not mandatory.
  • Physical exam within 6 months prior to start, including clearance of communicable diseases.
  • Immunizations or documented immunity required for:
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Measles, Mumps, Rubella (proof of immunity or vaccination)
    • Hepatitis B (titer, vaccination series, or declination)
    • Varicella (titer, vaccination, history of chickenpox, or declination)
    • Seasonal flu vaccine or declination during flu season (Oct 1 - Mar 31)
    • COVID-19 vaccine or approved medical/religious exemption
  • Negative TB test or chest X-ray within 1 year.
  • 9-panel drug screen within 1 year.
  • Auto insurance card required.
  • Two professional references required (travel/allied/per diem acute care).

Additional Information

  • Mileage reimbursed for travel between patient visits (not to/from home).
  • Contract length is 4-6 months; less experienced clinicians must commit to 6 months.
  • All required compliance documents must be submitted before starting patient care.
  • Requests for time off must be submitted with application; no additional time off requests accepted after offer.
