Job Opportunity: Licensed Practical Nurse (LPN)

Location: Rochester, New York
Employment Type: Travel/Contract
Contract Length: 8 weeks

Job Details

• Shift: Evening (3:00 PM - 11:30 PM)
• Patient Population: Mix of Long Term Care and Rehabilitation patients
• Average Daily Census: 120 patients
• Staffing Ratios:

  • Days: 1 nurse to 20 patients
  • Nights: 1 nurse to 40 patients
  • Weekends: 1 nurse to 20 patients
• Float Requirement: Must float between all 3 units in the building
• Weekend Coverage: Every other weekend (total of 8 weekend shifts during contract)
• Unit Accepts First-Time Travelers: Yes
• Local Candidates: Must live more than 75 miles away

Required Qualifications

• Valid LPN / LVN license in New York
• Minimum 1 year of Long Term Care experience within the last year
• ACLS issued by American Heart Association
• BLS issued by American Heart Association
• PALS issued by American Heart Association
• NRP certification
• TNCC certification
• EFM certification
• Tech Cert Verification - National
• Experience in a Long Term Care setting
• Orientation Safety Manual Acknowledgement
• State License 1 Verification
• NURSYS Verification (RN, LPN/LVN, AP)

Compliance and Documentation Requirements

• Government-issued photo ID
• 7-year criminal background check (including felony checks) within 3 months of start
• National and state-level background screenings (e.g., EPLS, HHS/OIG)
• Education verification from primary source
• State license primary source verification within 30 days of start
• NURSYS verification within 30 days of start
• Physical exam within 1 year of start, updated annually
• Fit mask certification within 1 year of start, updated annually
• Negative TB test or chest X-ray within 1 year of start
• Required immunizations or proof of immunity:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screening within 30 days of start
• Seasonal flu vaccination or signed declination during flu season
• Snellen eye exam within 1 year of start
• COVID vaccination copy preferred but not required
• Medication/pharmacology competency exam as required
• Color blindness testing (one-time)
• Competency exams as required by specialty
• Orientation safety manual acknowledgement
• Photo for badge (passport-style with solid background)
• References (2 within 3 years) required for agency direct hires
• Employment application and work history verification

State-Specific Requirements (New York)

• NYS Nurse Aide Registry verification within 3 months of start (for LTC and related care)
• Fingerprint-based criminal background check may be required depending on unit assignment
• Disciplinary action search results may be required depending on unit assignment
• Sex offender registry check within 30 days of start
• NY Medicaid Exclusion List check within 3 months of start

Additional Notes

• Prior travel experience is not required for this position.
• Candidates must be prepared to complete all health-related requirements and lab work within 5 days of signing the contract.
• Floating between units and weekend coverage are mandatory parts of the assignment.
