Job Opportunity: RN Home Health (Travel)
Location: Newton, Massachusetts
Contract Length: 13 weeks
Job Description
- Provide clinical services to patients of all ages in their homes.
- Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families.
- Perform admissions and case management in accordance with organizational policies and standards.
- Utilize thorough and timely electronic documentation and interdisciplinary communication.
- Must be willing to work weekends, holidays, and be on-call as needed (weekend rotations occur once every 6 weeks).
- Reliable, independent transportation required for home visits.
Required Experience
- Minimum 1 year of Certified Home Care experience.
- Minimum 1 year of Case Management experience.
Required Qualifications
- Current RN license in the state of Massachusetts (no conditional licenses accepted).
- Must have reliable transportation for home visits.
- Willingness to work weekends, holidays, and on-call shifts as scheduled.
Prior Travel Experience
- Not explicitly required.
Required Credentials and Compliance
- Color photo (recent, professional headshot).
- Motor Vehicle Record (MVR) check within 45 days of start.
- Primary source verification of RN license within 30 days of start.
- N95 fit test within 365 days of start, updated annually (no exemptions allowed).
- Nurse Aide Registry check within 30 days of start (required for all employees).
- Documentation of MMR vaccination or immunity.
- Influenza vaccine required during flu season (no exemptions).
- TB screening (skin test or blood test) within 3 months of start, updated if >6 month break.
- Health statement signed by MD/DO/NP/PA within 365 days of start.
- Criminal background checks including Massachusetts CORI, SSN trace, national sex offender registry, and exclusion checks within 45 days prior to start and updated as required.
- Documentation of education verification (highest level achieved).
- Respirator Medical Clearance attestation if fit test required.
- Resume including 7 years of work experience with city and state.
- Two professional references within 4 years, including at least one supervisor.
- Skills checklist completed within 1 year of submission.
- Confirmation of assignment form upon acceptance and updates if start date changes.
Additional Information
- Must be willing to comply with all vaccination and health screening requirements.
- No religious or medical exemptions allowed for N95 fit testing and influenza vaccine.
- TB positive individuals must provide chest x-ray documentation showing no active tuberculosis.
This position is ideal for an experienced RN with home health and case management experience who is comfortable working independently in a travel assignment setting.
