Inpatient Pharmacist Position
Location: Salem, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Details
- Schedule: 40 hours per week, rotating day and evening shifts
- Shift Times:
- Day shift: 7:00 AM to 3:30 PM
- Evening shift: 2:00 PM to 11:30 PM
- Unit: Inpatient Pharmacy
- Patient Population: Newborn through geriatric, including medical and behavioral health patients
- Electronic Medical Record (EMR) System: Epic
- Equipment: Omnicell medication dispensing system
Required Qualifications
- Valid Pharmacist license in Massachusetts
- Pharmacy degree
- Minimum 1 year of pharmacist experience within the last year
- Experience in an inpatient setting
Onboarding and Compliance Requirements
- Professional Documentation:
- Resume including past 7 years of work experience with city and state
- Two professional references within the last 4 years (minimum one supervisor-level)
- Skills checklist completed within 1 year
- Confirmation of assignment form upon acceptance and for extensions
- Licensing Verification: Primary source verification of pharmacist license required within 30 days of start date; conditional licenses not accepted
- Background Checks:
- Criminal background check within 45 days prior to start and updated if break >6 months
- Massachusetts CORI check within 30 days prior to start and updated if break >6 months
- Social Security Number trace for last 7 years
- National sex offender registry check within 30 days prior to start and updated annually
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start and annually
- Health Requirements:
- Documentation of MMR immunity (vaccines or blood test)
- TB screening: Negative TB skin test or blood test within 3 months of start, updated if break >6 months
- Physician health statement within 365 days of start, updated if break >6 months
- Hepatitis B vaccination or proof of immunity strongly preferred
- Tdap vaccine within last 10 years preferred
- Additional: Name change documentation if legal name differs from credential documentation
