Physical Therapist Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: 8:00 AM – 4:30 PM
Job Summary
Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.
Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Ability to travel to patient homes as assigned
Required Credentials & Compliance (Must be completed prior to or at onboarding)
- Professional license verification and primary source verification within 30 days of start date.
- Criminal background check, including Massachusetts CORI and national sex offender registry, within 45 days prior to start and updated if there is a break in service.
- Motor Vehicle Record (MVR) check within 45 days prior to start (required for all home care positions).
- Resume including past 7 years of work experience with city and state details (required for submission).
- Two professional references within the last 4 years, including at least one supervisor or higher in the skillset (required for submission).
- Skills checklist completed within 1 year of submission (required for submission).
- Confirmation of assignment form upon acceptance and for any extensions.
- Education verification of highest level achieved (verified through background check).
- Social Security Number trace as part of criminal background check.
- Exclusion checks against OIG, GSA, and state Medicaid exclusion lists within 30 days prior to start and annually.
- Nurse Aide Registry check within 30 days of start (required by Massachusetts Department of Public Health for all employees).
Health & Safety Requirements
- N95 respirator fit test within 365 days of start date and updated annually (no exemptions allowed). Fit testing cannot be done onsite.
- Respirator Medical Clearance attestation completed prior to fit testing if applicable.
- Documentation of immunity or vaccination for Measles, Mumps, and Rubella (MMR) via vaccines or blood titer.
- Tuberculosis (TB) screening within 3 months of start date, either by skin test or blood test, with updates if there is a break in service longer than 6 months.
- Physician-signed health statement within 365 days of start date and updated if there is a break in service longer than 6 months.
- Optional: If positive TB test, documentation of a negative chest x-ray after the positive test.
Additional Notes
- Experience in hospice, private care, or case management alone does not meet the home health experience requirement.
- Weekend rotations are infrequent (once every 6 weeks).
- Must be able to travel to patient homes as assigned.
