Key Requirements and Credentials

Licenses & Certifications

• Nursys Verification: Must be completed within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions.
• Current Nursing License: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
• BLS Certification: Must be American Heart Association (AHA) certified. Required before onboarding.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
• Driver’s License/State ID: Must be current, match address on file, and show front/back.

Testing & Exams (Required Before or At Onboarding)

• Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years.
• Ascension EKG Exam: Minimum 80% score, max 2 attempts, updated to latest version as required.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max 2 attempts, updated yearly.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
• Ascension Skills Checklist: Clinical skills checklist, completed within one year of start and updated annually.

Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses four weeks apart required.
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider. No declinations accepted. If negative, two doses four weeks apart required.
• Tdap: Valid within 10 years. No declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Declinations accepted but mask required at all times.
• 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd PPD within 30 days of start, or negative IGRA within 90 days. TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year.
• Fit Test: Required for all travelers, preferred before start; can be completed onsite (except PAPR). Annual update required.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified drugs. Retesting required if break in service > 90 days.

Onboarding & Documentation

• Ascension Resume/Profile: Required for submission.
• Two Professional References: Required prior to first assignment.
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
• Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire module with attestation upload.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updates for extensions.
• Ascension Employment Verification: Verification of past 7 years employment, updated annually or after break in service > 90 days.
• Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; updated if break in service > 90 days.
• Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service > 90 days.
• Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.

Optional

• Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
• Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended if applicable.
• Hiring Manager Evaluation: Post-assignment evaluation.
• Ascension Name Change Document: Legal documentation of name change if applicable.

This position requires strict adherence to credentialing, testing, and health requirements prior to and during the assignment. The start date is fixed, and the contract length is approximately 13 weeks.
