Role Summary
Provide direct physical therapy care, including evaluation, treatment planning, implementation, progress tracking, discharge documentation, and coordination with the therapy team.
Required Qualifications
- Valid Physical Therapist license in New York
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS
Key Responsibilities
- Evaluate patient needs and monitor progress
- Document progress and discharge summaries
- Maintain treatment records and statistics
- Coordinate therapeutic services with the therapy team
- Follow hospital and departmental policies, quality improvement, safety, environmental, and infection control standards
- Support family members in helping with therapy outside the hospital when needed
- Provide services for infant, pediatric, and adolescent patients
Physical Demands
- Prolonged standing, reaching, and walking
- Stooping, bending, and lifting or carrying up to 75 pounds
