Physical Therapist

Overview

• Provide restorative and rehabilitative physical therapy services to participants in a PACE setting, including care at the center and in participants’ homes.
• Work with an interdisciplinary team to support participant independence, safety, function, care coordination, and transitions of care.
• This role includes direct patient care, home visits, and participation in interdisciplinary meetings.

Schedule

• Monday through Friday, 8:00 AM to 4:30 PM
• Combination of on-site and mobile work based on participant needs
• 1–2 days per week dedicated to home visits

Required Qualifications

• Bachelor’s degree in Physical Therapy
• Valid California Physical Therapy license
• BLS certification
• Five years of progressively responsible physical therapy experience
• Two years of physical therapy experience in a clinical setting with a frail or elderly population
• Two years of supervisory experience
• Legal authorization to practice in the job location

Preferred Education

• Master’s degree in Physical Therapy or Doctorate in Physical Therapy

Work Environment

• PACE setting
• Reliable transportation is required for the mobile and home-visit portion of the role
