Registered Nurse – Home Health

Location: Green Bay, Wisconsin
Employment Type: Travel Contract
Contract Length: 13 weeks
Shift: 8-hour or 10-hour shifts (flexible up to 12 hours)
Guaranteed Hours: 36-48 hours per week (minimum 36)
Weekend/Holiday Rotation: Every other weekend (open to every weekend), up to every other holiday

Job Overview

Provide home health nursing care within a 9-county radius in Northeast Wisconsin.
Average travel distance: 75-100 miles per day, with 4-6 patient visits daily (travel time included in shift).
Work independently in patients’ homes, caring for a wide variety of patients from newborns to seniors.
Care for patients recovering from illness, new diagnoses, injury, or surgery, aiming to help them return to prior or improved function.
Multidisciplinary team environment including nursing, therapy, social work, and CNA staff.

Required Qualifications

Valid RN license in Wisconsin
Minimum 2 years of Home Health nursing experience, including experience within the last year
Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Experience with Epic electronic health record system
Basic Life Support (BLS) certification issued by the American Heart Association
Experience working in a Home Health setting
