Physical Therapist - Long Term Contract

Location: Dallas, California
Contract Length: 13 weeks

Job Details

• Schedule: Monday to Friday
• Guaranteed 40-hour work week
• Coverage area includes multiple nearby territories within approximately a 60-mile radius
• Average 4-6 patient visits per day (PT/OT/SLP)
• Productivity expectations: 28-32 points per week

  • SOC: 2.5 points
  • ROC: 2 points
  • Evaluations/Recert/Discharge OASIS: 1.5 points
  • Discipline DC, routine visit, 30-day reassessment: 1 point
  • Weekly case conference: 1 point
• Overtime requires pre-approval
• 2 weeks cancellation notice required

Requirements

• Home Health and OASIS experience required (no exceptions)
• Homecare Homebase (HCHB) documentation experience required
• Client may consider candidates without HCHB experience at a reduced rate for the first 2 weeks if manager training is provided
