Job Description
Join the Hospice team supporting patients and families through the final stages of illness. Services are provided in home settings including patients' residences, skilled nursing facilities, assisted living facilities, and adult foster homes. The role involves intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support.
Required Qualifications
- Valid LMSW (Licensed Master Social Worker) license in Oregon
- Minimum of 1 year of LMSW experience within the last year
- Current Healthcare Provider Level BLS certification
- Experience in Home Care and Home Health settings
- Valid driver’s license and personal automobile with required insurance; vehicle must be maintained in good working order
