Job Opportunity: Licensed Clinical MSW (Behavioral Health)

Position Summary

• Serve as an Outpatient/Inpatient Social Worker responsible for direct supervision and support of high-risk patients.
• Design and manage a continuum of care to empower clients toward self-sufficiency and demonstrable outcomes.
• Conduct assessments, service planning, resource acquisition, progress monitoring, and respond to emerging client needs.
• Link clients with community social services, healthcare providers, substance abuse, and mental health resources.
• Collaborate with a multidisciplinary team to promote efficient use of healthcare resources and provide seamless care.
• Work closely with providers, nurses, case managers, physicians, and other healthcare staff to implement appropriate resources.
• Evaluate patients in the Emergency Department for medical necessity and care coordination needs.
• Assess potential admissions and coordinate outpatient plans for safe discharge with physician approval.
• Coordinate care for patients with abuse, chronic pain, and frequent utilization issues.
• Interact with patients of all ages, including those with mental health issues, to help them access and navigate resources.

Required Qualifications

Education

• Master’s degree in Social Work (MSW)

Licensure/Certification

• Valid Licensed Clinical Social Worker (LCSW) license in Arizona
• Licensed Clinical Social Worker with a minimum of 3 years of experience, including experience within the last year
• Licensed Master Social Worker (LMSW) experience
• Current Basic Life Support (BLS) certification issued by the American Heart Association

Experience

• Experience in discharge planning, case management, or utilization review processes

Additional Information

• Orientation occurs every 2 weeks
• COVID & Flu vaccination exemptions may be accepted after submission of a declination form
• Housing application must be submitted 2 weeks prior to start date (shared contractor housing only; no pets or family allowed)
