Registered Nurse (Home Health RN) - Travel Assignment

Location: Red Bluff, California
Contract Length: 13 weeks

Job Overview

Seeking an experienced Home Health RN for a travel assignment providing care primarily to recently discharged adult patients at high risk for rehospitalization. Patient population includes those with CHF, COPD, wounds (including wound vacs), and IV-PICC lines. No pediatric patients.

Work Schedule

• 5 days per week, 8-hour day shifts (40 hours/week)
• Typical schedule: Monday-Friday or Tuesday-Saturday (preference for Tuesday-Saturday)
• Holiday coverage needed (Thanksgiving and/or Christmas)
• No call or standby requirements
• Floating within Home Health only

Patient Load & Duties

• Approximately 5 visits per day
• Visit types and point values:

  • Start of Care (SOC): 2.5 points
  • Recertification: 1.5 points
  • Resumption of Care: 1.75 points
  • Routine visits: 1 point
• Patient education related to respiratory therapy equipment (vents, CPAP, BiPAP) as needed
• Utilization review involvement not specified

Required Qualifications

• Minimum 2 years of Home Health RN experience
• Active California RN license
• BLS certification required
• Valid driver’s license and insurance
• Experience with HomeCare HomeBase (HCHB) preferred

• Mileage reimbursement per GSA guidelines
• Dress code: any work-appropriate scrubs
• Home health aide support available in the department

• Manager available during working hours for routine questions; administrator on call after hours and weekends
• Schedule may change day-of due to start of care processing or sick calls
