Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care (Evenings)

Location: Claremont, New Hampshire (Rural area, approx. 2 hours from Boston, MA)
Employment Type: Temporary
Shift: Evenings
Hours per Day: 8
Contract Length: 13 weeks


Job Overview

  • Provide continuous observation of residents, noting changes in mental and physical well-being and communicating with supervisors and physicians as needed.
  • Administer medications and treatments as ordered by physicians, documenting accurately using electronic systems.
  • Supervise Licensed Nursing Assistants (LNAs) according to New Hampshire Board of Nursing regulations.
  • Assist with nursing care and assign resident care duties consistent with staff abilities and scope of practice.
  • Accompany physicians on resident rounds and facilitate communication during 60-day reviews.
  • Administer oxygen therapy and gastrostomy/jejunostomy feedings per facility policies.
  • Perform wound care, including knowledge of wound vacs.
  • Monitor unit activities such as therapy appointments, ambulation lists, vital signs, and nutritional documentation.
  • Oversee infection control practices and maintain a clean, hazard-free environment.
  • Admit, discharge, and transfer residents with proper documentation.
  • Complete 24-hour reports, review new orders, and maintain communication with nursing leadership.
  • Communicate with residents’ families regarding care concerns and obtain consents as necessary.
  • Manage assigned unit during shift under direction of Nursing Supervisor or Director of Nursing.
  • Follow state and federal regulations for controlled medication storage, usage, and documentation.
  • Assist with orientation, education, and evaluation of nursing staff; recommend disciplinary actions as needed.

Required Qualifications

  • Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in good standing.
  • Minimum of 3 years of current experience in nursing, with at least some experience within the last 6 months.
  • IV certification required (must be willing to obtain during onboarding if not already certified).
  • COVID-19 vaccination required; no declinations or exemptions accepted.
  • Willingness to float to other units where experienced.
  • Must have own transportation due to rural location and limited public transit.
  • Locals residing within 50 miles of the facility are not eligible.
  • Previous travel nursing experience is highly preferred.
  • Experience in rural or small facility settings is highly preferred.

Additional Requirements

  • Provide a current resume.
  • Submit a skills checklist completed within the last year.
  • Provide two supervisory references from the last two years of employment.
  • All certifications must be active at time of submission.
  • Must comply with facility dress code: matching scrubs top and bottoms.
  • Must adhere to facility policies regarding overtime, callback, and cancellation.

  • Facility provides 24-hour nursing care in a long-term care setting.
  • Emphasis on preventive and restorative care addressing physical, emotional, spiritual, and psychological needs.
  • Collaborative care planning involving residents and families is encouraged.
  • Facility is Medicaid licensed and Medicare approved as a Skilled Nursing Facility.
  • Orientation hours are unpaid.
  • Timekeeping is electronic; no paper timecards will be signed by managers.
  • Annual seasonal flu vaccine required or declination with mask-wearing during flu season.

  • This is a rural assignment with limited housing options; candidates must plan accordingly.
  • Holiday time off requests are not approved; time off requests cannot exceed 5 days.
  • Locals within 50 miles will not be accepted to ensure travel nurse placement.
