Job Description

• Provide home health physical therapy services, primarily geriatric cases
• Average patients per day: 5-7 (to be confirmed during interview)
• Guaranteed 40 hours per week
• On-call required every 3rd to 4th weekend
• Territory covers up to a 60-mile radius from the hub/office
• Dress code: Scrubs (any color), no jeans
• Use of iPad provided; cell phone use not allowed during work
• Mileage reimbursed at IRS standard rate
• Flexibility with territory expected
• Timely documentation and communication required

Requirements

• Minimum 1 year recent experience in Home Health PT
• OASIS experience required
• Experience with HCHB EMR required
• Must have current state license in hand
• BLS (Basic Life Support) certification required
• First-time travelers accepted
• Locals accepted at travel rate

Certifications & Compliance

• BLS (AHA, ARC, Military acceptable)
• Orientation Safety Manual Acknowledgement (annual update)
• State License verification (primary source within 90 days of start and upon expiration)
• Medication/Pharmacology Competency Exam (as applicable)
• Competency Exam (as applicable)

Health & Safety Requirements

• Physical exam within last 12 months
• Negative TB test or chest X-ray (within 1 year; chest X-ray within 5 years if positive PPD)
• Annual TB questionnaire
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within last 12 months
• Flu seasonal vaccination or facility-specific declination form during flu season
• COVID-19 vaccination or medical/religious exemption, including booster

Background & History Checks

• Background check within 90 days of start, updated annually, including:

  • SSN trace
  • County searches for last 7 years
  • FACIS Level 1
  • Nationwide sex offender check
  • Motor vehicle records
  • Education and work history verification
• EPLS/GSA/SAM check within 90 days of start
• Two professional references within 2 years of start
• Full 7 years of work history with no gaps required on application
• Skills checklist required annually
• Fingerprinting requirements vary by state (e.g., CT, OH) and must be current within 12 months
• State-specific searches as applicable (e.g., Kentucky, South Carolina, Missouri, Florida, Virginia)

Additional Facility-Specific Requirements

• Proof of current auto insurance (required for all Home Health positions involving driving)
• Completion of Acceptable Use Policy and Agency Background Attestation prior to start
• Flu and COVID statement form if declining flu vaccine
• Missouri Crime Disclosure Form (for MO placements)
