Job Overview

• Location: Smithtown, New York
• Employment type: Travel/Contract
• Shift: Night, 3x12 hours
• Hours per week: 36
• Contract length: 13 weeks

Required Qualifications

• Valid RN license in New York
• Associate of Science in Nursing / Associate Degree in Nursing
• Psychiatric experience within the last year
• At least 1 year of psychiatric experience in the specialty or in a comparable or higher-acuity setting
• 2 clinical references at supervisor level or above; charge RNs are acceptable
• Driver’s license or state ID with address and clear picture
• Current skills checklist, completed within the last year and applicable to the position
• 7 years of employment history with any gaps explained
• A current and valid phone number and email address on the application

Required Certifications and Testing

• BLS issued by American Heart Association
• State license verification within 30 days of start
• Pass/fail testing, including medication/pharmacology and dysrhythmia/EKG competency as applicable
• Acknowledgement of pass/fail testing
• Medication test/pharmacology competency exam
• Dysrhythmia/EKG competency exam

Required Health and Compliance Items

• Background check completed within 30 days of start and updated annually
• EPLS/GSA/SAM check within 30 days prior to start
• HHS/OIG check within 30 days prior to start
• New York State Medicaid Exclusion List check within 30 days prior to start and updated annually
• Physical exam using the facility-specific form, initiated within 24–48 hours of offer acceptance and completed within 30 days prior to start
• Fit test completed within 12 months of start
• Negative TB test or chest X-ray completed within 1 year of start
• Hepatitis B antibody and antigen results
• Measles, mumps, rubella, and varicella immunity or vaccination documentation
• Standard 9-panel drug screen completed within 30 days prior to start
• Flu vaccination or approved declination/exemption during flu season
• COVID vaccine documentation is preferred if available

• The physical exam cannot be completed onsite.
• Rapid drug tests are not allowed.
