Certified Occupational Therapy Assistant (COTA) - Travel/Contract

Location: Burbank, California
Assignment Length: 13 to 26 weeks preferred (approximately 13 weeks minimum)
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Schedule Details:

May include occasional weekend coverage and floating as needed
Cancellation notice: 14 days

Workweek: 36 hours minimum guaranteed

Required Qualifications

Valid Certified Occupational Therapy Assistant license in California
Minimum 1 year of Certified Occupational Therapy Assistant experience within the last year

Compliance and Documentation Requirements

Background checks including:
- 7-year location history search
- Sex offender registry check
- EPLS/GSA/SAM verification (annual)
- HHS/OIG verification (annual)
Competency exams as applicable, including medication/pharmacology test
Facility-specific compliance quiz on wage and rehab hours (California)
Physical exam within 1 year of start date
Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
9-panel drug screen within 1 year of start date
Seasonal flu vaccination or declination during flu season (October 1 - March 31)

References and Skills

Two professional references required for travel, allied, or per diem acute care roles (within 3 years)
One professional reference required for per diem non-acute care roles
Annual skills checklist required
