Job Opportunity: Certified Occupational Therapy Assistant (COTA) - Skilled Nursing Facility
Location
- Lancaster, California

Assignment Details
- Start Date: ASAP
- Contract Length: 13 to 26 weeks preferred
- Weekly Hours: 36 to 40 hours
- Shift: Day shift, 5 days per week, 8-hour shifts (8:00 AM to 4:30 PM)
- May include occasional weekend coverage and floating as needed
- Cancellation Notice: 14 days
Required Experience
- Minimum 1 year of experience in specialty
- New graduates will be considered
Required Certifications and Licenses
- Current state license as a Certified Occupational Therapy Assistant (COTA)
- ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
- BLS (Basic Life Support) from American Heart Association or American Red Cross (current and clear copy)
- PALS (Pediatric Advanced Life Support) preferred
Required Documentation and Compliance
- Government-issued photo ID (Driver’s License or State ID)
- Emergency Contact Form (required prior to start)
- Background checks including:
- Standard 7-year search of locations lived/worked/school
- Sex offender registry check
- EPLS/GSA/SAM (annual)
- HHS/OIG (annual)
- Physical exam within 1 year of start date
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 1 year plus annual TB questionnaire
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
- Drug screening: 9-panel drug screen within 1 year of start (instant drug screen acceptable)
- Seasonal flu vaccination or declination required during flu season (October 1 - March 31)
- Completion of facility-specific compliance quiz related to wage and rehab hours (prior to start)
Competency Requirements
- Medication Test/Pharmacology Competency Exam (as applicable)
- General Competency Exam (as applicable)
- Annual skills checklist
References
- 2 professional references required for travel, allied, or per diem acute care roles
- 1 professional reference required for per diem non-acute care roles
- References should be within 3 years of start date
