Job Details

• Specialty: Home Health Physical Therapy
• Shift: Day shift, 5 days a week (8:00 AM - 4:30 PM)
• Hours: 40 hours per week, 8 hours per day
• Patient Load: Average 5-7 patients per day
• Productivity: Minimum 6 daily visits or 30 points weekly
• Case Types: Geriatric patients
• Territory: Up to 60-mile radius from office, with occasional extensions based on patient needs
• Dress Code: Scrubs of any color; jeans not allowed
• Device Provided: iPad
• Cell Phone Use: Not allowed for work purposes
• Mileage Reimbursement: Provided at IRS standard rate
• Guaranteed Hours: 40 hours per week
• Flexibility: Traveler expected to be flexible with territory coverage
• First Time Travelers: Not accepted

Required Qualifications

• Valid Physical Therapist license in Washington
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification

Compliance and Background Requirements

• Background checks must be current within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education and work history verification (7 years of work history required with no gaps)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
• State-specific criminal history and employability status checks as applicable
• Professional references: 2 within 2 years of start (travel, allied, per diem acute care)

Health and Safety Requirements

• Physical exam within the last 12 months (not required annually)
• Fit test for N95 mask (no specific model required)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required for all candidates
• Immunizations or titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required
• 9-panel drug screen within 12 months of start (instant acceptable)

Additional Requirements

• Proof of current auto insurance required due to driving responsibilities
• Signed acknowledgment of Orientation Safety Manual and Acceptable Use Policy prior to start
• Agency background attestation required before onboarding
• Compliance with facility-specific forms and documentation as applicable
