Job Opportunity: SLP Home Health (Travel/Contract)
Location: San Jose, California
Position Overview
- Specialty: Therapy/Rehabilitation (Speech-Language Pathologist and Physical Therapist roles available in various counties)
- Employment Type: Travel/Contract
- Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
- Visits: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
- Patient Load: Focus on patients per week rather than per day
Experience Requirements
- Minimum 2 years overall clinical experience
- Minimum 1 year experience in Home Health
- First-time travelers accepted (no prior travel experience required)
Contract Details
- Contract Length: 4-6 months (less experienced clinicians must commit to 6 months, non-negotiable)
- Weekend Work: Not required
- On-Call: Not required (discretionary)
- Locals Accepted: Yes, within a 50-mile radius
Licensure & Certifications
- Must have valid State License at time of submission (no pending licenses accepted)
- Required Certifications:
- BLS (Basic Life Support)
- ACLS (Advanced Cardiac Life Support) preferred
- PALS (Pediatric Advanced Life Support) preferred
- Compliance documents must be complete before starting (may take 2-3 weeks)
Work Environment & Policies
- Dress Code: Any color scrubs, closed-toe shoes
- Charting: Must be completed onsite using Kinnser/Wellsky; no charting from home allowed
- Mileage Reimbursement: IRS standard from first patient to last patient (not to/from home)
- Parking: Free residential parking available
- COVID & Flu Policy:
- COVID vaccine required (declination allowed for religious reasons)
- Flu vaccine optional with declination form required
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License/State ID)
- Background checks within 1 year, including SS Trace, County searches, Nationwide Sex Offender check
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Physical exam within 6 months signed by MD/DO, including statement of no active communicable diseases
- Immunizations and titers as applicable:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella proof or declination (for candidates born 1957 or later)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, history, or declination)
- Drug screening: 9-panel drug screen within 1 year
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
- COVID vaccination or medical/religious exemption documentation
- Emergency contact form required prior to start
- Auto insurance card required for all candidates
Additional Notes
- Territory/County coverage will be discussed during interview; may include surrounding counties
- Clinicians valued for being patient care focused, punctual, self-starters, and diligent with charting
- References:
- 2 professional references for travel, allied, or per diem acute care
- 1 professional reference for per diem non-acute care (not required for non-clinical roles)
