Job Details

- CPI (Crisis Prevention Institute) certification (if applicable).
Required Background Checks & Verifications
- Background check within 45 days of start, covering all locations lived/worked in past 7 years.
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly.
- Education Verification for nursing degree.

- Negative TB test or chest X-ray within 12 months, updated annually.
- MMR (Measles, Mumps, Rubella) vaccination or positive titer with declination option.
- Hepatitis B vaccination or signed declination.
- Varicella vaccination or positive titer with declination option.
- Seasonal flu vaccine required during flu season or declination accepted.
- COVID vaccine or declination required.
- Respiratory safety clearance for respirator use, medically cleared annually.
Competency & Skills Requirements
- Medication Test/Pharmacology Competency Exam as applicable.
- Competency exams as required by specialty.
- Color Blind Testing if required by unit.
- Skills checklist updated annually for applicable units.
Facility & System Specific Requirements
- Completion of Providence Health and Services WorkRamp Core and Specialty Modules prior to start.
- Epic access training modules required for Per Diem candidates in SoCal and other specified regions.
- Photo for badge with neutral background may be required.
- Float policy acknowledgment: May be required to float to other Providence facilities within reasonable distance.
