• Shift: Monday to Friday, 8:00 AM – 5:00 PM (5x8's)
• Area of Coverage: San Jose/South Bay/Santa Clara County
• Locals Accepted: Within 50 miles (at local rate)

Required Qualifications

• License: California Physical Therapy Assistant (PTA) license required
• Certifications:

  • PTA Certification
  • Basic Life Support (BLS) – AHA or ARC only
• Experience: Minimum 2 years of experience in specialty (Therapy/Rehabilitation)
• Prior Travel Experience: Not explicitly required

Background and Compliance Requirements

• 7-year background check required within 6 months of start
• Motor Vehicle Report required (due to driving position)
• FACIS Level 3 screening required within 2 weeks of start and monthly thereafter
• Education verification required (highest level achieved)
• Sex offender search required within 90 days of start
• Various federal and state sanction list checks required and updated annually
• Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)

• Immunizations and health screenings required:

  • Tetanus/Diphtheria/Tdap (within 10 years)

  • Measles, Mumps, Rubella, Varicella, Hepatitis B immunization or titers as applicable
  • Seasonal flu vaccination during flu season (Nov-April) or signed declination
• Respiratory safety clearance required, renewed annually
• Personal or auto insurance required for driving positions, effective for assignment duration
