Occupational Therapist (SNF) - Travel/Contract

Location: Live Oak, California
Assignment Length: 13 to 26 weeks
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP

Job Description

• Provide occupational therapy services in a Skilled Nursing Facility (SNF) setting.
• May include occasional weekend coverage and floating as needed.

Required Qualifications

• Valid Occupational Therapist license in California.
• Minimum 1 year of Occupational Therapist experience, with experience within the last year.

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID).
• Emergency Contact Form completed prior to start.
• Background checks including 7-year residence/work/school history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG checks (updated annually).
• State License verification via primary source.
• National or state-specific tech certification as applicable.
• Medication Test/Pharmacology Competency Exam and other competency exams as applicable.
• Facility-specific compliance quiz related to wage and rehab hours.
• Physical exam within 1 year of start date.
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest x-ray within 1 year and annual TB questionnaire.
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine series, or history of chickenpox)
• 9-panel drug screen within 1 year of start (instant drug screen acceptable).
• Seasonal flu vaccination or declination during flu season (October 1 - March 31).

References & Skills

• Two professional references required for travel, allied, or per diem acute care roles; one professional reference for per diem non-acute care roles (within 3 years of start).
• Annual skills checklist required.

Additional Notes

• This is a travel/contract position requiring flexibility for occasional weekend shifts and floating duties.
• Candidates must comply with all health, background, and competency requirements prior to start.
