Registered Nurse - Hospice (Travel/Contract)
Location: Fresno, California
Contract Length: 13 weeks
Job Overview
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Work involves home visits with periodic office meetings.
- Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
- Willingness to travel up to 75 miles from the home clinic; approximately 300 miles traveled weekly.
- Weekend work required every weekend (2 weekend days included).
- On-call duties required; specifics vary and should be discussed during interview.
- Shift: Day shift, typically 5x8 hours (8:00 AM - 5:00 PM), some positions may have 4x10 shifts (Friday-Monday).
- Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
Required Qualifications
- Valid RN license in California.
- Minimum of 1 year of hospice nursing experience within the last year.
- Experience working with adult patients in a home health setting.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Current BLS certification issued by the American Heart Association or American Red Cross.
- Proficiency with HomeCare HomeBase (HCHB) electronic medical records.
Work Environment & Expectations
- Dress code: Scrubs.
- Productivity expectation: 25 weekly units.
- Patient travel compensation: Paid from patient home to patient home; commute to/from first and last patient is not billable.
- Local candidates accepted at the same rate.
- Weekend and on-call requirements vary; discuss during interview.
Health & Compliance Requirements
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases; updated annually.
- Negative TB test or chest X-ray within 12 months, updated annually; results cannot be pending at start.
- Proof of immunizations or titers for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 vaccination or declination.
- Seasonal flu vaccination or facility-specific declination documentation required.
- 10-panel drug screen per Aya Healthcare standards.
Background & Documentation Requirements
- Standard background check covering the last 7 years of residence, work, and education history.
- Various state-specific background and fingerprinting requirements depending on placement location (e.g., PA, FL, NC, MO, SC, VA).
- Education verification required; transcripts or diplomas accepted if degree verification is unavailable.
- Two professional references required.
- Work history verification for at least two most recent employers or last 7 years.
- Completion and signing of multiple compliance documents including confidentiality forms, HIPAA and privacy policies, code of conduct, compliance training, and facility-specific policy acknowledgments prior to start.
- Must complete all required state and facility-specific releases, attestations, and background screenings before starting assignment.
Additional Notes
- No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
- Must be willing to comply with all agency and state-specific compliance and documentation requirements.
- Candidates must sign new supplier packets at the start of a new assignment and annually if working more than one consecutive year.
This position is ideal for experienced hospice RNs with recent specialty experience who are comfortable with travel and home health environments.
