Required Qualifications

  • Active RN license with verification
  • BLS certification
  • Chemotherapy/Biotherapy certification

Onboarding Requirements

  • COVID-19 vaccination documentation, declination, or exemption
  • MMR immunization documentation
  • Varicella immunization documentation
  • Hepatitis B series documentation or declination
  • TB screening: 2-step PPD or IGRA (Quantiferon/T-Spot); chest X-ray and TB questionnaire if prior TB result is positive
  • Fit test within 1 year of start
  • Physical/health statement from an exam within the last year
  • Trinity 10-panel drug screen completed within 30 days of start
