Job Opportunity: Nuclear Medicine Technician

Location: Plymouth, Indiana
Employment Type: Long-Term Assignment (approximately 13 weeks)
Work Schedule: Primarily Monday through Friday at one location, with occasional assistance at a nearby campus as needed

Required Qualifications

• Valid Nuclear Medicine Technologist license in Indiana
• Minimum of 1 year of experience as a Nuclear Medicine Technologist within the last year

Onboarding and Compliance Requirements

Candidates must provide the following documentation, all current and valid as specified:

Certifications and Licenses

• BLS certification (front and back images)
• License/certification verification dated within 30 days of start

Health and Immunizations

• Fit Test
• Annual TB Questionnaire
• 10-panel drug screen completed within 30 days of start (specific substances tested, no THC)
• COVID-19 vaccine proof including booster or approved declination/exemption (with agency letterhead)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: At least first vaccination in series prior to start
• Influenza vaccine or declination for current flu season
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella: Proof of vaccination or IgG titer lab report
• Physical/Health Statement confirming fitness to work within the last year
• Tdap vaccine or declination if no pediatric interaction

Professional and Background Checks

• Signed Code of Conduct form
• Completed I9 form
• Confidentiality and System Security Agreement
• Integrity and Audit Services Form signed shortly before start
• Joint Commission CORE Exam within the last year
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years) including SSN verification, dated within 30 days of start
• Education Verification (highest academic degree or equivalent)
• EPLS (Excluded Parties List System) report dated within 30 days
• OIG (Office of Inspector General) exclusion report dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• OFAC (Office of Foreign Assets Control) sanctions check dated within 30 days
• System Access Form completed and uploaded one week before start
• Skills Checklist provided by agency for clinical contractors
