Schedule

• Night shift: 7:00 PM - 7:30 AM

Contract Length

• 13 weeks

Required Qualifications

• Valid RN license in Ohio
• Med Surg experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License
• Active permanent RN license; temporary licenses are not accepted
• BLS certification from the American Heart Association
• Driver’s license or state ID with a non-expired photo ID; address must match the candidate profile
• Skills checklist completed within 1 year prior to start
• License verification through Nursys and primary source verification
• Ohio fingerprints and criminal background screening
• Employment verification within the last 5 years
• Education verification
• Competency exam with a passing score of at least 80%
• Core competencies with a passing score of at least 80%
• Submission packet including resume, 2 professional references from the last 2 years, and candidate-completed job application
• Prior W-2 employment form
• I-9 attestation or completed I-9 on file
• Physical exam statement showing ability to work without restrictions
• Fit test completed within 1 year prior to start
• MMR immunity documentation or required booster process
• Varicella immunity documentation or required booster process
• Hepatitis B documentation, series, or signed declination
• Tdap vaccination within the last 10 years
• TB testing within the last 6 months
• Drug screen within 60 days of start
• Background check within 6 months of start
• OIG search within 6 months prior to start
• SAM, EPLS, GSA, and OFAC screening within 6 months prior to start
• Safety and compliance training modules completed prior to clearance

Additional Notes

• Flu vaccine proof is required from October through March; declinations require masking while working
• COVID-19 vaccination is preferred but not required
• If applicable, Ohio residents may need a national database criminal background check
• If applicable, a prior positive TB test requires a clear chest x-ray and current TB questionnaire
