• TB Testing: Positive IGRA or PPD results, chest X-ray within one year showing no infection, and TB questionnaire completed. Annual updates required.
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider. No declinations accepted. If negative, two doses of vaccine required four weeks apart.
• Tdap: Valid within 10 years. No declinations accepted.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• Driver’s License/State ID: Must be current and valid; address must match home state for compact license holders.

Professional Credentials & Documentation

• Resume/Profile submission required.
• At least two professional references required prior to first assignment.
• Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and other checks within 30 days of first assignment; updated annually or after breaks in service >90 days.
• EPLS check within 90 days of start; rerun if break in service >90 days.
• Skills checklist to be completed within one year of start and updated annually.
• 12 Panel Drug Screen within 30 days prior to start and updated annually.
• Education verification within 90 days of start; official transcripts required if verification fails.
• Confirmation of assignment documentation required upon initial assignment and updates with extensions.
• Employment verification for past seven years; updated annually or after breaks in service >90 days.
• Badge photo with white or blue background required.
