Schedule

• 2:00 PM to 12:00 AM
• Friday through Monday
• First week of training: 8-hour day shifts

Required Qualifications

• Valid LMSW - Licensed Master Social Worker license in California
• LMSW - Licensed Master Social Worker (2 years of experience)
• LMSW - Licensed Master Social Worker experience within the last year
• Master's degree in social work
• Experience with legal holds
• Master of Social Work experience

Preferred Qualifications

• Previous counseling experience

Key Responsibilities

• Complete initial assessments for patients on legal holds
• Document social determinants of health for admitted patients
• Assess mental status, living situation, support system, substance use history, mental health history, physical functioning, financial status, and discharge planning needs
• Complete and respond to social work consults related to PCLS issues
• Communicate patient legal hold status and expiration timing with nursing management
• Coordinate discharge planning with unit social work/case management for psychiatric-related services and legal hold transitions
• Maintain collateral contact with families and agencies for discharge planning and fact gathering
• Identify outpatient psychiatric follow-up needs
• Monitor legal hold cases through status transitions
• Support conservatorship referrals when indicated

Skills and Abilities

• English communication, reading, writing, and speaking
• Ability to use computers, printers, software, and basic office equipment
• Ability to type 30 words per minute accurately
• Ability to work independently, prioritize workload, and manage multiple assignments
• Strong interpersonal, leadership, collaboration, and problem-solving skills
• Ability to work calmly under pressure and maintain attention to detail
