Schedule
- Monday–Friday, 8:00 AM–5:00 PM
- No call
Role Overview
- Provides direct hospice patient care and supportive support to families
- Manages patient clinical records and coordinates care with the interdisciplinary team
- Supervises licensed practical nurses and hospice aides
- May participate in the agency on-call schedule
- This is a safety-sensitive position
Required Qualifications
- Active nursing license in Virginia
- Registered Nurse education
- Minimum of 1 year of RN experience in acute care hospital, oncology, long-term care, hospice, or home health
- BLS certification from the American Heart Association
- Valid driver’s license
- Current automobile insurance
- Ability to maintain transportation for work
Key Skills and Experience
- Hospice care and hospice philosophy
- Patient assessment and case management
- Care of terminally ill patients and families
- Documentation and program management
- Knowledge of community resources
- General nursing practice and managed care principles
- Ability to use nursing assessment tools, computers, facsimile equipment, copier, and cell phone/beeper
Education
- Approved four-year high school course of study or equivalent
- Diploma or degree from an approved professional nursing education program
Additional Notes
- Preferred software familiarity: HCHB
- Typical caseload: 5–6 patients per day
- Visit types include routine, PRN, and recertification visits
- Travel radius: 60 miles from the branch
