Job Opportunity: Temporary Registered Nurse (RN) - Home Health

Location: Kāne‘ohe, Hawaii
Employment Type: Temporary

Job Description

• Provide excellent patient care through collaboration and coordination of case management programs.
• Act as a liaison between patients, families, nurses, physicians, multidisciplinary teams, and healthcare benefactors to optimize patient outcomes.
• Serve as a consultant to the healthcare team on specific patient care issues.
• Lead coordination of patient care with other disciplines, ensuring appropriateness and timeliness of care.
• Monitor interdisciplinary care plans to align with patient clinical course, continuing care needs, and covered services.
• Discuss resource utilization, consultations, treatment plans, length of stay, and discharge plans with physicians.
• Focus on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients.
• Collaborate with acute care case managers to ensure ongoing care appropriateness.
• Coordinate patient transfers to tertiary centers, ensuring continuity of treatment and services.
• Provide oversight and collect data for regulatory and accreditation compliance.
• Manage frequent emergency department visitors by reviewing previous and current admissions.

Contract Length

• 13 weeks

Requirements

• Experience:

  • Minimum 2 years of nursing experience (required)
  • At least 6 months of home health experience (required)
• Travel Experience:

  • Prior travel experience is required
• Licenses & Certifications:

  • Active and clear driver's license and car insurance (required)
  • Hawaii nursing license (required)
  • COVID-19 and flu vaccinations (required, no declinations accepted)
• Skills & Knowledge:

  • Familiarity with Homecare Home Base and OASIS systems (required)
• Education:

  • Bachelor's degree preferred
• Additional Preferred Experience:

  • Case manager experience preferred

Note: Mileage reimbursement is provided at a rate lower than the IRS established rate.
