Case Manager (Registered Nurse) – Travel/Contract
Location: Tucson, Arizona
Contract Length: 13 weeks
Job Summary
- Responsible for all aspects of transitional plan of care from admission to discharge.
- Conduct initial case management assessment interviews and provide transitional care follow-up as needed.
- Ensure safe discharge to patient’s final destination (home, self-care, home health, skilled nursing facility, inpatient rehab, etc.).
- Collaborate with community partners and vendors.
- Must multitask, prioritize, document accurately, and coordinate care effectively.
Work Schedule
- Monday to Friday, 8:00 AM – 4:30 PM (40 hours/week)
- Weekend coverage required at least once every 4 weeks (minimum 3 weekend days during the 13-week contract), with a weekday off for compensation.
Required Qualifications
- Valid RN license in Arizona.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 2 years of case management experience, including acute hospital case management.
- Case management experience within the last year.
- Minimum 2 years or more of total travel experience.
- Experience working with adult patients in inpatient settings, including Emergency Department (ED) experience.
- Proficient with Epic Electronic Medical Record system.
- Knowledge and experience in utilization review (InterQual) and discharge planning.
- Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Current Neonatal Resuscitation Program (NRP) certification.
- Current Advanced Cardiovascular Life Support (ACLS) certification.
- Current Pediatric Advanced Life Support (PALS) certification.
- Electronic Fetal Monitoring (EFM) certification.
- Crisis Prevention Institute (CPI) or Management of Assaultive Behavior (MAB) certification.
Clinical & Job Skills
- Knowledge of Medicare/Medicaid/insurance coverage is helpful but not required; resources available for learning.
- Ability to assess behavioral health cases and collaborate with specialized behavioral health teams.
- Proficient in EPIC EMR and Microsoft Office Suite (Teams, Outlook, Word, Excel, PowerPoint).
- Familiarity with Health Stream, ACMA, Strata Health, and UKG platforms is beneficial.
Compliance & Documentation Requirements
- Must provide current driver’s license or state ID with correct name documentation if applicable.
- Background checks including 7-year history, county/state searches, EPLS/GSA/SAM, and HHS/OIG must be completed within 30 days of assignment start.
- Physical exam within 1 year of start, signed by physician, NP, or PA.
- Fit testing for N95 masks within 1 year of start.
- Immunizations and health screenings required:
- Tetanus/Diphtheria/Tdap (within 10 years; declinations allowed except for certain pediatric and maternity units).
- Negative TB test or chest x-ray within 3 months of start.
- Proof of immunity or vaccination for Measles, Mumps, Rubella, Varicella (declinations not accepted).
- Hepatitis B vaccination or positive titer (declinations allowed with signed form).
- Seasonal flu vaccination required during flu season (Oct 1 – Apr 30) or approved medical/religious exemption with facility-specific form.
- Drug testing (12 panel) must be completed within 30 days of start.
- Pass/Fail testing for EKG and medication competency required for certain units.
- Annual competency exams including medication test, HIPAA, and specialty-specific exams with passing score of 80% or higher.
- Crisis Prevention Institute (CPI) or Management of Assaultive Behavior (MAB) certification required.
Additional Requirements
- Must provide 2 professional references from employment within the last 3 years, including at least one supervisory reference.
- Employment application with detailed 7-year work history and explanation of any gaps over 30 days required.
- Completion of facility-specific orientation and acknowledgment forms required prior to start.
- Adherence to facility tobacco, nicotine, and vaping policy.
Notes
- Mental health support/therapy is provided by specialized teams; case managers must assess and refer appropriately.
- Candidates must be comfortable working with a high population of behavioral health cases in collaboration with specialists.
