Physical Therapist Assistant (Home Health) - Travel/Contract

Location: Morehead, Kentucky
Contract Length: 13 weeks
Shift: Day (Monday-Friday, 7:30 AM - 4:00 PM)
Hours: 40 hours per week (5 days x 8 hours)

Job Summary

Provide skilled physical therapy interventions under the supervision of a licensed Physical Therapist to assist patients in improving mobility, strength, balance, endurance, safety, and functional independence within their home environment. Travel throughout Lewis and Carter Counties to deliver patient care in homes and community settings. Collaborate with an interdisciplinary home health team to optimize patient outcomes.

Key Responsibilities

• Deliver physical therapy interventions according to the established plan of care.
• Monitor patient response to treatment and communicate progress and concerns to the supervising Physical Therapist.
• Educate patients, families, and caregivers on home exercise programs, safety strategies, fall prevention, and adaptive equipment use.
• Maintain accurate, timely, and compliant documentation per agency, state, federal, and payer requirements.
• Participate in staff meetings, case conferences, quality improvement initiatives, and continuing education.

Required Qualifications

• Valid Physical Therapy Assistant license in Kentucky.
• Current Basic Life Support (BLS) certification.
• Valid driver’s license.

Compliance and Documentation Requirements

• Government-issued photo ID must be carried during hours of service.
• Background checks: 7-year criminal history check within one year of start, updated annually; must cover all locations lived/worked in past 7 years.
• Required federal exclusions and sanctions screenings (EPLS/GSA/SAM, HHS/OIG) within 30 days of start and updated with contract renewals.
• Drug screening: 9-panel drug test within the last 12 months (instant acceptable).
• Physical exam: Initial and annual updates attesting ability to meet physical demands of the position.
• Immunizations and health screenings:

  • Negative TB test or chest x-ray within 1 year (or chest x-ray within 2 months if TB positive).
  • Tdap vaccine or declination.
  • Measles, Mumps, Rubella (titer or 2 vaccines).
  • Hepatitis B (titer, vaccine series, or declination).
  • Varicella (titer or 2 vaccines).
  • Seasonal flu vaccination required during flu season (no exemptions).
  • Pneumococcal vaccination or declination as applicable.
• Color blindness testing required.
• Completion of competency exams and medication/pharmacology testing as applicable.
• Compliance with attendance, professional image, hygiene, and code of conduct policies.
• Proof of two clinical references or hospital evaluations.
• Completion of employment application/resume and skills checklist updated annually.
