Job Opportunity: Physical Therapist - Home Health

Job Description

Seeking a Physical Therapist with home health experience to provide skilled therapy visits in patients' homes. Patient population includes medical/surgical, cardiac, orthopedic (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, stroke, falls, weakness, Parkinson’s, respiratory patients, trauma, amputees, and more.

Typical caseload: 6-8 home visits per day, with a minimum of 6 points per day (points system managed by supervisor). Expect 1-2 weekends per contract. Visits and hours are self-scheduled based on office needs.

Required Qualifications

• Valid Physical Therapist license in Florida
• Basic Life Support (BLS) certification
• Minimum 1 year of experience in Physical Therapy Home Health
• Experience in a Home Health setting
• Orthopedic therapy experience

• Use of equipment and devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, supplemental oxygen, prosthetics, wheelchair assessment)
• Orthopedic and neurological therapy
• Home exercise program development
• Medication knowledge
• Patient and family education
• Recognition of signs and symptoms of physical, sexual, and psychological abuse
• Safety and injury prevention
• Billing knowledge

Additional Requirements

• Physical exam within 6 months of start
• Negative TB test or chest X-ray within 10 weeks of start (or chest X-ray and TB questionnaire if prior positive TB)
• Annual TB questionnaire completion
• Required immunizations or titers: Measles, Mumps, Rubella, Hepatitis B, Varicella (with boosters if needed)
• Standard 9-panel drug test within 30 days of start
• Respirator medical clearance if respirator use is required
• Background checks including education verification and fingerprinting per Florida AHCA Level II standards
• Completion of required competency exams and medication/pharmacology tests as applicable
• Completion of facility-specific online training modules prior to start
• Professional photo for badge
• Current personal/auto insurance coverage
