Med Surg / Telemetry (1 year of experience)
Med Surg / Telemetry experience within the last year
ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
BLS - Basic Life Support issued by American Heart Association or American Red Cross
PALS - Pediatric Advanced Life Support issued by American Heart Association or American Red Cross
NIHSS

Government-issued photo ID
Background check, including 7-year criminal search, SS trace, county searches for the last 7 years of locations lived/worked/in school, nationwide search, sex offender search, and Pennsylvania statewide patch search
EPLS/GSA/SAM check
HHS/OIG check

2 professional references within 3 years of hire
Skills checklist
Annual competency exam and medication/pharmacology competency exam as applicable

Tdap vaccination within the past 10 years
Measles, mumps, rubella, hepatitis B, and varicella immunity/documentation as specified by facility policy
