Job Description

• Coordinate the overall interdisciplinary plan of care for patients from admission to discharge.
• Develop care plans across the continuum, integrating patient and family preferences and values.
• Monitor patient medical necessity and level of care through assessments, ongoing evaluations, and patient records.
• Advocate for resources and removal of barriers to care.
• Maintain ongoing communication with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans.
• Serve as a resource for medical record documentation, level of care recommendations, and post-discharge care services related to diagnoses and treatment options.

Required Specialty Experience

• RN Case Manager (Years of experience not explicitly stated; assume standard RN Case Manager experience expected)

Required Certifications and Licenses

• BLS (American Heart Association)
• Current nursing license with Nursys verification within 30 days prior to start
• License/Certificate verification within 30 days prior to start
• Ascension-specific competencies and onboarding requirements (including color vision test, core competencies, unit-specific checklist, and onboarding attestations)

Health and Safety Requirements

• Proof of two MMR vaccines or positive IGG titer (no declinations accepted)
• Fit Test (preferred prior to start, can be completed onsite; updated annually)
• Documentation of Hepatitis B vaccination series or signed declination
• Influenza vaccine (seasonal; exemptions accepted with mask requirement)
• TB screening: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire
• Health statement or physical exam prior to first assignment, updated annually
• Documentation of Varicella immunity or vaccination (no declinations accepted)
• Tdap vaccination valid within 10 years (no declinations accepted)
• 2-step PPD or IGRA testing within specified timeframes prior to start
• 12-panel drug screen within 30 days prior to start, updated annually

Professional and Background Verification

• Resume/profile submission required
• At least two professional references prior to first assignment
• Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender check, and other screenings within 30 days of first assignment
• EPLS (Excluded Parties List System) check within 90 days of start
• Verification of education relevant to clinical work within 90 days of start
• Employment verification for the past seven years
• Confirmation of assignment documentation required upon initial assignment and updates for extensions
