Job Details

  • Employment Type: Long-Term Assignment (LTA)
  • Contract Length: 13 weeks
  • Shift: 7:00 AM – 5:30 PM
  • Weekly Hours: 40 hours per week
  • Schedule: Variable weekly schedule with every other Saturday coverage including on-call
  • Coverage primarily for multiple facilities; must be willing to assist at other locations when census is low
  • Reliable transportation required

Required Qualifications

  • Valid RN license in Ohio
  • Minimum of 1 year of dialysis experience
  • Dialysis experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification issued by the American Heart Association
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Driver's License

  • Specialty: RN Dialysis

Onboarding and Compliance Requirements

  • Primary source verification of RN license and Nursys verification within 30 days prior to start; must be updated every 2 years with license renewal

    • Ohio fingerprinting and FBI check depending on residency duration in Ohio
    • OIG exclusion check within 6 months prior to start
    • SAM/EPLS/GSA/OFAC screening within 6 months prior to start
  • Drug screen within 60 days prior to start (10-panel, lab report required; instant screens not accepted)
  • Verification of highest education level through third-party; international education requires equivalency documentation
  • Employment verification for last 5 years through third-party
  • Completion of clinical skills checklist within 1 year prior to start (one-time requirement)
  • Competency exam for relevant department with minimum 80% score within 1 year prior to start (one-time requirement)
  • Completion of core competencies (HIPAA, OSHA, Joint Commission topics) with minimum 80% score within 1 year prior to start (one-time requirement)
  • Completion of required safety and compliance training modules prior to clearance; renewal annually
  • Submission packet required at application including resume, 2 professional references (within last 2 years), job application, and employment history form

Additional Notes

  • Must be willing to work every other Saturday and be on-call as scheduled
  • Must be flexible to assist at other facilities as needed based on census fluctuations
