Minimum Qualifications
<ul><li>Associate degree in Physical Therapy from an accredited educational program.</li><li>Current, active West Virginia PTA license required for the assignment.</li><li>Experience in home health care required. Must be comfortable providing care in patients' homes and traveling between patient visits.</li><li>Experience with HCHB documentation preferred.</li><li>BLS certification from the American Heart Association or American Red Cross may be required.</li><li>Minimum education requirement: Associate degree.</li><li>Preferred attributes: flexibility, strong communication, adaptability, and compassion.</li></ul>
 
Assignment Details
<ul><li>Facility type: Home Health.</li><li>Shift name: Days.</li><li>Shift schedule: Monday–Friday, 08:00–16:30 (days).</li><li>Weekly guaranteed hours: 40 hours per week.</li><li>Productivity expectations: 25 points.</li><li>Coverage area: Grant, Hardy, and Pendleton Counties, WV. Travel between patient homes required. Mileage reimbursed between first and last patient visit.</li><li>On-call/weekend requirement: No routine on-call or weekend requirement.</li><li>Dress code: royal blue scrubs.</li></ul>
